For me (I'm still a young fellar), it would have to be Oregon Trail on the old Apple II computers in second and third grade of elementary school during the early 1990's. Later when two games called Doom and Doom II were released in 1993/1994, my PC gaming life took off from there.
I have other early memories with various FPS from Apogee Sotware during the mid-90's, now known as 3D Realms; I remember juggling the various floppy disks for installation of Blake Stone: Aliens, also don't forget Wolfenstein 3D, Duke Nukem and even Rise of the Triad.

A multitude of DOS games, too many too list
You said "PC games", so I didn't think at first that you were including the 8-bit micros from 30 years ago. My first such game would be Star Raiders on the Atari 400.
Hmm. That was a while ago, in the mid 90s. Probably Supaplex, Biomenace or plain old Tetris.
It's been a long time since I've played Oregon Trail (my oxen died like mad back then) -- that was probably the first PC game I played. I think I played it at summer school during my kindergarten/first grade years.
I played Wolf3D when I was 4, that is the first game I can remember playing, was more focused on treasure hunting than killing nazi's and their dogs.

My first games... 'Putt-putt', 'Where in the world is Carmen Sandiego' and my favourites 'Lucasarts Adventure Games: Monkey Island, Full Throttle and Day of the Tentacle'. My uncle gave them to me, and I've loved them ever since.

Wolfenstein when I was 3-4ish years old where by at the end of my first gaming session I told my father.

Well, I can't recall the titles of my first two games, so I'll have to list my third: Oregon Trail. The old one, the one that was just purple on black.

Fisher Price Bowling and Fire Rescue on two 5 1/2" floppy disks. I was very young, I can't remember when it was.
Does Granny's Garden count? Far, far scarier than Doom. If not, then er, Doom.
Whizz Kid for the Amiga. Awesome game, I still play it from time to time. Never completed it though.
I don't even remember what it was called. I just remember my grandma having this awesome state-of-the-art computer (which now makes me lawl) that had some games on it. It was some top-down adventure type thing; like Zelda but with worse graphics.
I don't remember the first one specifically, but the first games in my "cycle" were Commander Keen, Hocus Pocus, OMF: 2097 and Wacky Wheels. I still occasionally play OMF: 2097 with DosBox. What's great is that a lot of these DOS games are now classified as abandonware, and can be legally downloaded for free off of the Internet.

Ignoring the boring old PC games from Elementary school... Either The Sims or Sim Hospital. And to date those are still the only PC games I've ever played.

Not true. It's about as legal as ROMs are. In other words, despite being widely available on the internet and having the myth that they're free floating about, it is illegal to download the software without paying for it. Sure, most of the time, nobody does anything about it, but as far as international copyright laws are concerned, it is illegal.
